Health care staff can refer or patient families can self-refer to Project HEAL
If you are a health care staff making a referral, there are no specific referral forms for Project HEAL. You make contact the Project HEAL director via telephone or email and provide the following information for a referral:
- Child’s Name
- Parent’s Name
- Telephone Number
- Issue (housing, special education, etc.)
Patient families may also contact the Project HEAL director via telephone or email.
To receive assistance from Project HEAL, the patient must receive on-going medical or mental health services from the Children’s Center
Patient families must meet the income guidelines of Project HEAL. Household income may not exceed 50% of the Maryland median income. Income eligibility is determined by the Project HEAL director
